It's Just A Matter Of Time

The headlines, that presumably describe the determination of probable cause, frequently do a real injustice to individuals and organizations, and most importantly, obscure the root cause of an aviation accident or incident. Every occurrence regardless of seeming simplicity, is the result of multiple causes or factors. Similarly, every human error, regardless of how grievous, is a product of multiple causes and factors. These multiple causes and factors that lead to an occurrence are usually latent and active failures that are within an organizational and cultural context. Without identifying these and taking corrective action or control, it's just a matter of time.

Beacon Field Corporation is an independent organization of professionals, who through our Independent Aviation Safety Evaluation process, can examine and identify trends and/or existing latent and active failures within an organization. We look beyond the specific standards and review the entire organization as it currently exists and offer recommendations to prevent occurrences and manage risks.

active failure - is an error or a violation which has an immediate adverse effect.

latent failure - is a result of a decision or an action made well before an accident, the consequences of which may lie dormant for a long time. Such failures usually originate at the decisionmaker, regulator, or line-management levels, i.e. with people far removed in time and space from the event.
